Crisis Coordination Intervention
This program offers intensive service coordination for children with special health care needs that are Medicaid Medically Fragile, Technology Dependent, Chronically Ill and have a Developmental Disability. We will engage in a collaborative effort in seeking the services that may enhance the quality of life for both the child and family. The service coordinator will work with the family to access eligible services and resources needed to maintain the child at home. The service coordinator will work to research and explore existing opportunities to meet the unique and specific needs of the child and family. There will be an emergency “team” if necessary to support the parents. The “team” will be comprised of a nurse, care manager, family support coordinator and supervisor staff.
Eligibility Criteria
This program offers intensive service coordination to families of Medically Fragile, Technology Dependent, Chronically Ill and Developmentally Disabled persons
families who reside in Nassau and Suffolk Counties.
Will help children from birth through 21 years of age
